This website links to an online appointment system for scheduling. Information entered there is handled by that scheduling system and the practice, not by this website.

The site does not require an account, and it does not ask you to upload medical records, images or test results.

Please do not send detailed medical records, imaging, test results or other protected health information through a web form or by ordinary email. Records are transferred through an approved, HIPAA-compliant channel arranged by the practice.

If you have already sent something you are unsure about, contact the office and it can be handled appropriately.

Protected health information held by the practice is governed by the practice’s Notice of Privacy Practices under HIPAA, which sets out how your information may be used and disclosed and what rights you have over it.

That notice is provided by the practice and takes precedence over anything on this page in relation to your medical record.

Where analytics are used, they are configured to measure how the site is used rather than to identify individuals.
