De Quervain's Tenosynovitis: Causes, Symptoms and Treatment

De Quervain's tenosynovitis causes pain and tenderness on the thumb side of the wrist. The tendons that move the thumb pass through a narrow compartment, and swelling can make their movement painful.

Lifting, gripping, wringing and repeated thumb motion often bring out the symptoms, but the condition can also occur without an obvious change in activity.

What it feels like

  • Pain over the thumb side of the wrist
  • Tenderness that may extend toward the thumb or forearm
  • Pain with lifting a child, opening a lid or moving the thumb across the palm
  • Swelling, creaking or reduced thumb use because movement hurts

Evaluation

The diagnosis is usually clinical. Examination compares thumb and wrist motion, localises the tenderness and checks for arthritis, a wrist problem or another tendon condition. Imaging is used when the pattern is unclear or another cause is suspected.

Nonsurgical care

  • A thumb-and-wrist splint that limits painful tendon movement
  • Changing lifting and gripping technique while symptoms settle
  • Hand therapy for motion, ergonomics and gradual strengthening
  • An injection in selected cases after discussing benefits and risks

When release surgery is considered

If pain remains limiting despite nonsurgical care, releasing the tight tendon compartment may be discussed. The tendons are inspected and protected, and the return to activity is guided by healing and comfort.
